Tony,

Bob offered an easy solution. 

However, you can do this by using the proper laser print table. 

For sidebar. Use the landscape print code, set page and form length to 45 
Select 16 CPI font. 

For portrait, set form to 60,60 and then use the appropriate font. 

Stay away from the proportional fonts as these so add complications.
