5.0.14 - freechain?

Since this most likely is a *nix system, I would suggest looking at the hardware side of the issue.

I have a client where a free chain rebuild on one file is set off on one computer(windows system) each time(at least once a month).  Did a rebuild yesterday and about a month ago on this file.

So, is the issue happening on the same machine each time or is it random? I use the term machine as I do not know of this a terminal or a computer running a terminal emulator.
