Make a printcode

                              $1b &l2a0o6d0e66F

Put that on the F8 options page on the output format as the
form init.

If you are printing 3 different records per sheet, then define
the form as 22 lines in define output. 22/22

If each sheet contains only one record, then define the form as 66/66.

This assumes letter size paper.
