Yes, I usually see something like:
<offlist>
at the very top like you suggest. I went rushing back to read the original
and see how I could have possibly missed the note... and it was at the end
with the cc: I sort of stop reading messages when I hear the ending and the
cc stuff. Won't do that again. One of my absolutely unforgivable internet
behavior is when someone posts something which is either obviously *not* for
public consumption or is specifically marked "dont' make this public" or
whatever. Even though there was nothing bad or improper in the message, the
sender wanted it private and I really wish I had caught that.

INcidentally, there is one situation where I could care less about
private/public posting... it's when someone sends something obscene or
inflammatory into my personal mailbox thinking I won't let anyone else see
it... thereby protecting their gross indiscretion. Nope, stuff like that
goes right up! :-) Keeps the idiots in line.
