empty records?

I should have qualified that "only thing" with "outside of processing 
that is unintentionally adding records".

Since you said that has only happened recently, you may want to consider 
any new users you have that may be trying to select Add Records 
somewhere where  they are using a screen that is improper in the way I 
described below.
