file access problem

No idea.

Is it only this file/index combination?

Are they searching for the same, or different index values?  (And, if it's 
the same value, what happens if you try a different value?)

What if you run dclerk without any processing, and do the exact same thing?

If running without any processing works, then run it with processing, but 
turn the debugger on for the second session, and see what your processing is 
doing that causes the failure.
