This is still the same server where Apache stopped putting TERM in the
environment for no obvious reason.  That problem seems to be solved;
however, it looks like that it was masking whatever the current issue
is.

fpsupport has direct access to our test server, so they can catch
whatever they need and read error logs as they like.
