Just struggled with this kind of message and discovered it was really a Deleted 
Key Index Error.  Took weeks to figure it out, but it always crashed when it was 
updating data in a file and it changed the index data.  See if you are getting 
Deleted Key errors on the indexes from the live file.

I had the log file pin down the error to three lines of code and all it was 
doing was updating a particular file and doing a Write to that file(WHICH IS 
WHERE IT CRASHED).  If I went to the file directly and update the record - 
change the indexed data manually and hit escape I got the deleted key index 
error message.  That is what finally illuminated it for me.  Then I could figure 
out where I forgot to lock the record, and it has nearly disappeared now.

Hope that helps.  Just one more crazy Windows Error.
