Well, you wrote mostly the answer I was going to, so I'll just kibitz.

Yeah, but there's a fairly standard set of extensions...

Putty defaults to xterm, for which there is a standard fptermcap entry
as well, I believe (I don't think we added that); you *do* need to
manually set the keyboard map to xterm instead of its default.  That
entry also doesn't include PV, though if you put it in it appears to
work properly.
