All indexes (or perhaps all up to the old limit of 16, index P) are supposed
to, and normally do, appear.
I wouldn't know what might bugger it on windows. One thing of course I'd
check permissions on the index files same as on unix.
Does this happen to all files or just one? or some?
Also verify what qualifier you are in at the time of running the cabe
wizard. As well as any other PF* variables that might affect where cabe
looks for files.
If you start from the same shell prompt with whatever env already loaded,
then whatever that env may be, at least that will ensure both cabe and
dxmaint are using the same.
