There are a few things that cause messages to go to the moderator which are
under the control of the sender.

  1.  Sending messages from addresses other than the one to which you're
      subscribed.  Any messages from senders who are not on the list get
      forwarded for approval.

  2.  Sending messages that are HTML-only or multi-part with HTML have a
      good probability of getting high spamassassin scores which will get
      forwarded for approval.  If the score is high enough, the message
      will be dropped.

  3.  Occassionally messages will contain patterns in the headers or body
      that trigger some anti-spam code I wrote years ago when this was
      still a Majordomo list.  This code is pretty crufty, the patterns
      haven't been updated recently, and spamassassin probably does a much
      better job of catching spam.  One of these years I will probably
      either get rid of this entirely or at least empty the patterns,
      leaving the tests there in case I want to use them later.
