You might want to look at the Atramax servers which I have developed and
are being marketed by our largest ISP customer.

Typically SMTP password authentication is done to handle roaming users who
want to send through their home server, not from MTA to MTA when sending
mail to domains hosted by the receiving system. We generally set our
machines up using WHOSON which is essentially pop/imap before smtp in that
any system's IP that's authenticated to the local pop/imap server is
allowed to relay mail. I like this as (a) it doesn't require any special
setup on the client's mail program, and (b) a single WHOSON server can
support multiple pop/imap and SMTP servers.
In cases where any of our customers have mail delivery problems we
basically smart-path their servers through one of ours (e.g. they're on a
QWEest dynamic or single IP static DSL connection and AOL rejects because
they think they're residential broadband). We normally handle this traffic
on a port other than 25 to avoid problems with broadband providers that
block outgoing SMTP traffic other than to their mail servers.
