Although I can't address the needs of this person for a Windows platform, 
it leads me to...:

We have just added features to AnzioWin, our terminal emulation program, 
for outbound dialing. With a phone number showing on the screen, the user 
can hit a key (or toolbar button) that invokes a user-written macro that 
dials the phone number, allowing the user to pick up the call from a 
telephone set connected to the same line. It's quite easy to do. It 
appears to be possible on every modem we've tried, including 'Winmodems'.

We did this at the request of a particular customer. They are just 
implementing it now, and are quite pleased with it.

We also hope to add feature support for reacting to inbound calls, 
especially with CallerID. A macro could extract the CallerID number, and 
use it to look up a user's account. Or, the incoming call (or CallerID) 
could trigger a macro. This feature is partially finished; it works on 
some systems but not on others, for reasons as yet undetermined.
