In my ~/.muttrc file I have the following which substitutes both
the From: and Reply-To: headers.

my_hdr From: bill at celestial.com
my_hdr Reply-To: bill at celestial.com

I have a line in the ~/.muttrc file

source ~/.mutt_hooks

That file has lines like this which generate different From: and
Reply-To: headers depending on my sending addresses:

send-hook "~t sqlobject" my_hdr From: sqlobject at celestial.com
send-hook "~t sqlobject" my_hdr Reply-To: sqlobject at celestial.com

send-hook "~t hylafax.org" my_hdr From: hylafax at celestial.net
send-hook "~t hylafax.org" my_hdr Reply-To: hylafax at celestial.net
