It's a page format...

letter size
portrait orientation
6 lpi
0 lines top margin
66 print lines

So the printer doesn't page until 66 lines have been received, conveniently
the number of lines in TWO filepro 33 line forms.

The code cannot reside on the output format itself. It has to be the init
code on the options page. Because we are printing 2 forms on a single page,
any printcode is executed twice on the sheet. That wouldn't do.
