problem with large file adding records

Red Hat Linux 7.3

This file may not be large to some, but about 280000 records is the
largest for this client.
We can not add record. There is no error. If you try to add record via
a processing table, it just exits.
This is my df on this box
Filesystem 1K-blocks Used Available Use% Mounted on
/dev/md1 75671728 46409008 25418776 65% /

If you try adding records via 3- Add Records, it just exits.
I have tried using expanding records with a zero entry, thinking this
might reset something in the file. No change.
When I looked at the number of records that could be added using A - Add
Records, it says 0!
I have try to reruild a index and I get a "BixCreate failed".
I can add recods to smaller files using the 3 - add records at dclerk.
But even on the small files A - Add Records reports 0 records can be added.
Can anyone tell me what could be happening here.
